The minimum data necessary to register a death with the Single Medical Certificate of Death include:
Data of the deceased: full name, identity document, geographical place and date of birth, age at the time of death, sex, marital status, educational level, habitual or permanent residence, place of death, and nationality.
Data on the death: geographical place, date, and time of death; whether medical attention was received during the illness or injury that led to death; whether the attending physician provided care; causes of death; ICD-10 code; basic cause of death; indication of whether the death was accidental or violent; procedure performed; and indication of whether the deceased was a woman of childbearing age (10 to 59 years).
Data of the certifying person: full name, occupation, institutional seal, and date.
